DODGE COUNTY YOUTH HOCKEY TOURNAMENTS

TEAM CREDENTIALS PACKAGE

Each team participating in invitiational, district or state tournaments must present the following credentials package to the tournament Credentials Committee prior to participating in the tournament. Any team that does not have a properly completed credentials package is not allowed to participate in the tournament.

MINNKOTA DISTRICT TEAMS

• Player roster completely filled out including the signatures of all participants (PLAYERS AND TEAM OFFICIALS). This roster must be signed by an individual that is authorized to approve rosters.
• Team Membership Application Form and Red Insurance Card that is signed and stamped by the Registrar, or the cognizant Associate Registrar.
• The Red copy or photo copy of the IMR for each rostered player. Players registered and rostered via the Cyber Sports Registration System do not need IMR copies.
• A Birth Certificate for each rostered player.
• The Red copy or photo copy of the IMR for each rostered team officia. Team officials registered and rostered via the Cyber Sport Registration System do not need IMR copies.
• An approval letter from the Minnkota District Registrar for any player that is playing in a younger age group then is implicit based on the players birth date.

NON-MINNKOTA DISTRICT TEAMS

• Player roster completely filled out including the signatures of all participants (PLAYERS AND TEAM OFFICIALS). This roster must be signed by an individual that is authorized to approve rosters.
• Team Membership Application Form that is signed by the Cognizant Registrar of Associate Registrar.
• The red copy or photo copy of the IMR for each rostered player. Players registered and rostered via the Cyber Sports Registration System do not need IMR copies.
• A birth Certificate for each rostered player.
• The red copy or photo copy of the IMR for each rostered team official (in some cases the team manager may not have an IMR, this is OK). Team officials registered and rostered via the Cyber Sports Registration System do not need IMR copies.
